Promise Point Care is seeking a compassionate and dedicated Case Worker to support and empower youth in a structured, trauma- informed environment. The Case Worker will play a vital role in helping youth develop life skills, achieve personal goals, and successfully transition into independent living.
Key Responsibilities
Key Responsibilities
- Manage a caseload of youth and provide ongoing support services
- Develop and implement individualized service/treatment plans
- Conduct assessments and monitor client progress regularly
- Advocate for youth needs and coordinate with external agencies (schools, counselors, courts, etc.)
- Maintain accurate and up-to-date case documentation and reports
- Provide guidance, mentorship, and crisis intervention when needed
- Facilitate life skills development (education, employment readiness, coping skills)
Qualifications
- Experience in social work, human services, or working with youth
- Experience with at-risk youth preferred
- Strong communication and organizational skills
- Ability to build trust and maintain professional boundaries
- Compassionate, dependable, and team-oriented
- Ability to handle sensitive situations with professionalism

Compensation: $18.00 - $24.00 per hour
